Engine revs up to about 2500 stabilizes 1000 running the idle surges then dies

It's a 2005 Ford Taurus 3.0L. I recently replaced blown head gasket and head on bank 2. Car runs fine as long as I'm moving

Sounds like you may have an intake leak.

The P2195 it's for the Oxygen Sensor and is the result of the problem but not the cause.
