This code for the cam sensor on bank 2 could be the cause of the hard starting hot. If its losing signal when the truck is hot then the computer isn't going to set off the injectors and spark.

As for the transmission, I would start by checking the transmission fluid, if it's low or burned it could indicate a problem with the clutch discs.
