What would cause my 04 hemi 5.7 Number 7 push rod to fall off

Misfire code p0300 04 5.7 hemi. Found a mashed push rod on cylinder 7.

A misfire condition would do this what happens is when there is a misfire the lifter collapse this is where the oil that should solidify the lifter when it does not the spring gives ot due to the lifter
