What causes cooling fan to always run?

The cooling fan on my 2006 Saturn VUE always runs, even when cold. Relays check ok. Coolant level is good. When I disconnect the battery, system runs fine for a few hours, then fan comes on and stays on while car is running.

The code is for the engine coolant temp below threshhold, this is likely due to the fans constantly running. I would replace the coolant temp sensor to see if the fans stop running non stop.
