I have a 2005 mustang GT 4.6, I've had the p0349 and p0345 codes for a while. I found out I had a bad cam phaser so me and my buddy replaced both of them. I have replaced both cam sensors before and after replacing the phasers. But the same two codes come back. I tested the continuity on both connectors and its good. I also checked both vct solenoids, the solenoids themselves and the connectors. All good. The car runs good but I have noticed when I'm driving the car at low rpm's it will jerk a little kind of like a hesitation I guess. Not a lot but definitely noticeable and enough to piss you off. Around 2k rpm's it smooths out and full speed ahead with no problems. What else could be the problem? What should I turn my attention to and look at next?

This code and this condition is so common on the 4.6 engine, you would also code a P0021 and a P0022, this is not going to the timing chain, or the camshaft timing gear or the timing tensioner. what common is insufficient oil pressure cause this code, this condition is commonly seen when a head gasket is replaced,  Your going to have to put elbow grease to get to the bottom of this, Check the oil condition , drain it in a clean container, and note if there is any silverfish color to it, if its clean enough put the oil back in.  What you  are going to look for is the cam bearing caps the ones at the front of the camshaft. if those cap wear out, you can expect this condition. This is mostly caused by a internal oil leak at the cam bearing caps,
