I had a Cam Phaser failure on bank2 lost power so I hauled the truck home, truck has 200,000+ miles on it. I tore engine down found a bad phaser on bank 2, chain guides shattered and down in oil pan so I dropped oil pan cleaned it out, installed new chains, guides, tensioners, oil pump, phasers ( when I installed the new phaser on bank 2 cam shaft I noticed that the dow pin felt sloppy so I replaced the cam shaft on bank 2 ) new VCT solenoids, cam sensors, crank sensor. this truck went to Ford three different times for cam phaser noise and they said they replaced them every time but when I picked the truck up it was always noisy and knocking in the top end and they always said that all of these engines make noise so I just let it go because it seems like the customer doesn't matter to Ford anymore. back to the truck now. after I did all this work I start the engine and after warm up it is quieter than the day I drove it off the lot all you can hear when you raise the hood at hot idle are the injectors and a noticeable increase in fuel milage. the only problem is I keep getting codes P0345 and P0349 I live in Maryland and we have Emission testing every two years and my truck is over due for testing and won't pass with codes.
the cam sensor Ohms are in range and with key on I have .4 volts on connector

if anyone can help I would be grateful as I have tried everything I can think of and hoping someone has had this problem and will share what they did.

thanks William

P0345 Camshaft postion (CMP) sensor circuit malfunction (bank2)

P0349 Camshaft position sensor "A" circuit intermittent (bank2)

P0349 and P0345 When these appear the charging system needs to be checked, This is a very popular code symptom where the alternator diode or open phase connector causes these code to appear.TSB # 06-09-12. The fix is to replace the alternator

Thanks I was getting exactly what you said p0345-po349 I had bout 2 new cam position sensors and the check engine light would not go off even after un plugging the battery so I took the first 2 back thinking I had gotten band ones and they replaced them for me I installed the second 2 and the light was still coming on but after reading this question and your answer I know now it's the alternator thanks for your help
