Lean condition are set when you have more air than fuel ratio, These can come from a torn air duct from the MAF to throttle body.having torns or cuts which would take in false air and upset the fuel trim, If that area is secure with no vacuum leaks found perfrom a MAF cleaning,

 The MAF is located at the air filter housing, just as the air filter get plugged and dirty so does the MAF,perfroming a MAF helps restore fuel trim codes,
