Car keeps going into limp mode after jerking and bucking at low speeds

It has gradually gotten worse to the point that I can't just reset the code and drive for a couple of days.

Can you gather the code that was set, to help determine , it may help pin point the exaxt problem . This problem can originate from a malfunction input speed sensor or the output speed sensor, to even the transmission fluid temperature sensor.
