P0731 Nissan - 1999 altima

Question

what do I need to do to fix this problem. Fluid level looks fine and clean, however, it has not been changed in along time. symptom I am having is when put into drive it sometimes stals due to hesitation. if It doesn't stall have to accelerate slowly or it will stall.
