Dodge Avenger SXT 2008, 2.7L V6

codes:
P0340 - Camshaft Position Sensor Circuit Bank 1 Sensor 1
P0344 - Camshaft Position Sensor Intermittent Interruption Bank 1 Sensor 1
P0456 - EVAP System Small Leak

The check engine light first came on a few days ago, I was hoping it was just the gas cap, I refilled the car and the light went away. It then came on again today and I noticed on the freeway, once I hit 60-70mph the accelerator would no longer work, I pressed it all the way but the car was still slowing down, once it slowed down to 50-60mph, the accelerator would work again, but if I went above 60, it would again not work until it slowed down. A few times it would shudder/shake before the accelerator would stop working. At city speeds 45 and under, there is never a problem and it appears to be working fine.

The first 2 codes seem to indicate checking the camshaft sensor's voltages / replacing it but the 3rd code doesn't seem to be related so any suggestions on how to diagnose the problem?
