i get code1391 on my 2001 chrysler t&c only when engine is cold

Question

When i start my van it runs bad and wont even idle or drive without dying and we get code 1391 and we have just changed the crank and cam sensors 3 times and traced the engine for shorts and there are non but after then engine temperature gets to normal running temperature it runs smooth. What could be causing this could it be the temperature sensor? even though we don't get the code for that? or incorrect flywheel? or does it need to be synchronized since we changed all sensors and computers in the van and flywheel/pressure plate? when we changed everything a month ago it ran great for about 3 days and then it started this running rough until warmed up.
