What is P2454 code for a 2008 GMC Diesel

Question

My check engine came on today and the code reader gave me a P2454 I would just like to know what it is? I did clear the code but it just came back as soon as I did that

P2454 Diesel particulate filter differential pressure sensor circuit low voltage

If the DPF is overloaded with soot,this can set this code,do a exhaust particaulate cleaning ,If the cleaning has been performed, replace the exhaust differential pressure sensor.
