2010 hyundai santa fe geting code p2191

I get the code p2191, what do i do it fix that? and what does it mean?
The car dosent run properly and when driving when coming to a stop sometimes it randomly shuts off.

This code means the rear O2 sensor is not happy with the exhaust it sees after the catalytic converter. This can mean a rich or lean condition.

Possible causes are intake leaks,exhaust leaks, a problem with the PCV system. Since the car is not running well at stops, I would start by checking for intake leaks as the possible cause.
