95 Nissan Maxima is my transmission shot?

The car shifted extremely hard when i was driving and then wouldnt shift at all. obd reader showed p0734 code. I changed out the tranny fluid and no go. Replaced solenoid pack yesterday and still won't shift. Is my transmission completely shot?

Unfortunately, it's very likely the transmission internal clutches are worn out and either needs to be rebuilt or get a new or used transmission assembly.
