This is the sensor in the exhaust manifold or top of the catalytic converter. Most often the sensor itself is the cause of the problem. Sometimes it is a wiring issue, but for the most part it's the sensor. Take a look at the wiring harness and make sure there isn't any damage or spots that might have rubbed through the insulation.
