Engine fault light comes on, 30 or so min later goes off again

I have  ford Mondeo diesel. Fault P0490 shown on obd11.
The light comes on and most times goes off after I have travelled a few miles or so and re starting the engine. The car makes some clanging noise  and I know that the light is about to come on again. It also splutters in low gear on some occasions. Have just serviced the car and replaced fuel filter but the problem comes and goes. This has been going on for a couple of years and the garage says that I need a new egr.
Question. Do I have to change the complete egr and manifold or just the valve?

That correct, the best approach is the EGR.
