Replaced Heated O2 sensor light went out for a while then returned

P013A code heated sensor replaced light went out and returned.  What could be causing the sensor to get fouled?

Sensor 2 are the down stream 02 sensor, replacing the sensor will not resolve this issue it is just reporting the condition of event that is occurring in the engine , do a MAF (mass air flow ) cleaning, this code is set due to the MAF sensor is taking in greater amount of air than required, or not reporting correctly
