I have a code fuel bank too lean bank 1

Question

This code went off on my 2010 Mazda 3. I have no problems with the car and have cleared the code multiple times. I Checked for leaks with smoke and replaced the o2 sensor also cleaned the MAF sensor. Nothing. I just put seafoam in my gas tank thinking my fuel lines are restricted any help is appreciated

This is more a PCV hose issue, what happens it develops cracks and allow the false air in , It will not show on the smoke test, this hose collapse during engine on.
