My van is a 1991 Dodge B250, 318 engine; when the fuel gauge gets around half, it starts having problems going up hills; it starts sputtering and losing power; if I pull over for a few minutes, and turn it off (if it hasn't already died), it starts back up and is alright if no hills.

When the vehicle start to sputter and lose power you can expect all types of code condition to set. You have a fuel pump problem , what happens if the fuel pump strainer falls apart, this in turn give low fuel pressure and volume. unfortunately you need to replace the fuel pump with it. You can start by replacing the fuel  filter as the first step.
