P2198 - what does it mean,is there problem

both bank 1 and bank 2 reading biased stuck rich, what does this mean

P2198 - 02 sensor signal biased / stuck rich  Bank 2 Sensor 1

The 02 sensor is responsible for the fuel control, when it is commanding rich it is making a correction for a lean fuel trim. this can be from a vacuum leak on one side of the Bank , this code should not be the only code present, with a scan tool do a KOER (key on engine running) test, This would help identify what is causing one side of the Bank being too lean
