P2195 2012 Ford Fusion - vehcile suddenly stalls when the excelerator is pushed.

Less than a mile after filling up and the car stalls. After a few minutes, car runs fine. After about 10 more miles of driving symptoms return. Check engine light stays on now and I am getting a P2195 code.
