When there is a fuel trim malfunction it would also be accompanied by a rich condition or a lean condition. Most fuel trim malfucntion are usually cause by vacuum leak, vacuum leaks are air leak that is unaccounted for, Air that enter the engine is measure by the MAF (mass air flow) any air that gets past the MAF is unaccounted air, which is a vacuum leak and upset the fuel to air ratio. Another cause are collapse vacuum hose. such as the PCV connection This is a collapse PCV connection , PCV is positive crankcase ventillation, without a proper crankcase ventillation , positve crankcase build up is capable of causing oil seal to leak
Broken vacuum line upset the air to fuel ratio, which can lean out the system and depending on the componenet it is connected to fail to make that component operational
