How to diagnose whether the throttle body needs to be replaced or if it just needs a re-learn procedure?

if you have not cleaned the throttle body it would be a good place to start. than you would have to re learn the iddle.  the only way to relearn the iddle is with the nissan computer from the dealer. even than you may still end up replacing the throttle body. esiest way to clean the throttle body is to remove the intake boot/tube have someone in the vehicle  turn the ignition key to on (DO NOT START VEHICLE) put vehicle  in drive with  foot on brake pedal and press on gas pedal. the throttle plate will open, clean throttle as needed.
