How do I fix a p0175 code(too rich bank 2) on a 2000 Ford F150 5.4L?

The last time this happened I put a PCV hose on and it fixed it for a few months.

P0175 Fuel trim too rich

How does the PCV look like , the PCV hose probable collapse which lead to it needing replacement. A plugged PCV valve can cause this also.
