P0138 Toyota -vsc flashing, traction control,check engine?

Only code shown is P0138 but vsc light is flashing, check engine light is on steady and traction control light is also on steady. I replaced the rear O2 sensor and reset codes, the car will drive 40 miles or so and those three lights return. Any thoughts or ideas?

P0138 -02 sensor Circuit High Voltage  (Bank 1 sensor 2)

This code condition is for the catalytic monitor. being a circuit high indicates a failed connection at the 02 sensor or the 02 sensor itself.Any CK ENG LT will turn of the VSC and the ABS this is for safety reason. get the code for the ABS system it should help point a direction of what causes the VSC to flash
