Oxygen sensor on Bank 1 Sensor 1 (in the back of the engine near the firewall) is most likely bad, if there is not wiring issue and if the ECM is not bad.

you can also check the oxygen sensor relay (A/F relay)

 

You can check the ECM if you want (see picture below)
