What are the likely causes for a P1454 ODB II code for 2005 Honda Element?

gas tank pressure. take gas cap off and put it back on

The first thing I did was replace the gas cap but the issue still remains. The interesting thing is that the car only trips the OBD code when the gas gauge reads less than a 1/4 tank remaining and when it is above 85 degrees outside. The code clears when I fill the tank or when the fall season comes.
