The Engine Control Module (ECM) monitors all the time the fan control circuit (or any other circuit), in other words check the wires from the ECM to the fan. In order to monitor the circuit, the ECM sends voltage to the fan and checks the amount of voltage that returns back to the ECM (the monitor system can also be done with voltage drop). If the voltage that returns to the ECM is lower that then expected, the ECM will set a low circuit code.

 Sometimes the low voltage return can be caused by a defective component, short circuit or poor connection at the wire connectors.
