Question

What can cause the O2 sensors to go bad? I have replaced the same one 3x in the last 18 months
(Bank 2 Sensor 2) and have changed the other 3. I only put 20000 in that span.

Are you using the factory O2 sensor? check for exhaust or injectors leaks. Exhaust or injector leaks and low quality fuel can cause a premature O2 sensor damage.

Yes, I replaced with NGK.  I don't have any exhaust leaks.  Gas mile age is consistent 20-21, 24 -25 hwy.
Is there a way to check injectors?
