I have a 2000 Elantra. How do I correct P1624, and gear 4 ratio incorrect?

So far I've changed the switch on top of transmission and the inner/outer sensors. At the beginning of each day the car run normal. The longer I drive sometime it began to jerk into gear. When I slow down to come to a stop it sometime jerk before complete stop. The engine light goes off sometimes, then come back on. Can some1 help?
