I followed your video and the connector checks out okay with power on and also the Noid light flashes when I run the engine.
But still have the code and rough engine due to misfire.
I've replaced the coil twice.
What next?

Hi there,

Check for intake vacuum leaks. with the engine running spray some carb cleaner along the intake manifold. If it's leaking the engine should smooth out or the idle speed should increase.

If there isn't any change, you can try swapping the fuel injector with the one next to it. If the misfire moves with the injector, then replace the injector. If not, do a compression test to see if there is a problem inside the engine.

Thanks for the tips.
I tried the carb cleaner trick nothing changed.
I couldn’t swap the injector leads since #4 is too short to reach #3 but #3 can reach #4.
I cleared the codes and it still ran rough.
Put everything back together and the P0354 came back  Ignition Coil D primary/secondary circuit.

Could the ECM be bad?
When I was removing the #4 spark plug I did touch the main power with my extension wrench and sparked something.

The truck was running perfectly fine before I decided to replace all the spark plugs and coils.

Your thoughts?

Possible you got a bad coil, so go ahead and swap it with the one next to it and see if the code moves. I meant to actually take the fuel injector out and swap it with the one next to it. Since you have new coils, but if it was fine before hand, then it doesn't seem like the injector should be a factor. Try the coil swap and see if it moves. might have gotten a bad one. Also, make sure none of the wires pushed back out of the harness when you plugged it in. It's also possible the plug got dropped before you got it and its causing the issue. I would look at the plug or the coil as the issue since you didn't have any problems before you did the swap.
