focus 1.6tdci black smoke and power loss (p0234)

Question

Car: Ford focus c-max 2005 1.6tdci 80kw without dpf and the catalyc is empty.

Since i bought this car (1 month back) it is always black smoking heavily and turbo whines all time also when idling but not smoking when idling. One time when accelerated hardly then car went into limpmode and diagnostic showed p0234, p138B and u0073.
The p0234 can be errased but p138B is permanent. Sometimes i feel power loss without any warning lights or limp mode.

I have changed turbocharger but it didnt solved anything. Changed EGR and also no difference. Changed MAF and no success. Changed also oil, fuel filter and air filter.
Dual mass flywheel is making bad sound for additional info.
