P2096 Hyundai - azera with 26000 miles

check engine light is on, and codes are p2096 and p0139 has anyone had this, and if so what was your fix.

P2096 Fuel trim rear sensor control limit exceeded

P0139 -02 sensor (rear) slow response.

These code condition point to a degraded catalytic convertor, unless you have a exhaust leak near the rear 02 sensor or the sensor is not propely seated.
