2004 Muranono problems, engine light is on, codes P0725 and P0455; what does this mean?

My 2004 Murano is giving me no problems, but the engine light is on and I got a code of P0725 and P0455; what does this mean?
122K miles
New gas cap May 2013
Camshaft Position Sensor replaced June 2013

P0725 is for Crankshaft Position (CKP) Sensor not Camshaft Position Sensor that you replaced.

 

P0445 EVAP Control System Gross Leak Detected

You will have to get the EVAP system for broken,cracked or loose hoses first

Possible causes

 

- Missing or loose fuel cap

- Incorrect fuel filler cap used

- Fuel filler cap remains open or fails to close

- Foreign matter caught in fuel filler cap

- EVAP canister or fuel tank leaks

- EVAP system hose leaking

- Fuel tank leaking
